CSA has partnered with General Vision Services (GVS) to administer an enhanced annual optical benefit for all CSA members. We have worked closely with GVS to streamline the out-of-network reimbursement process and the reimbursement schedule. Members and their dependents are provided an optical benefit once a year. All CSA Members may elect to use a GVS provider or request an out-of-network claim form for reimbursement of up to $150 on any combination of eye exam, frames, lenses or contact lenses.
Please be reminded that the maximum reimbursement of $150 must be used during a single visit. (The plan doesn’t allow members to receive an eye exam at one location and then visit a different location for glasses or contact lenses.) If you choose an out-of-network provider, send an itemized receipt, indicating that all services occurred during a single visit, to GVS with a claim form for reimbursement.

The CSA Welfare Fund is pleased to supplement the GVS-managed reimbursement with up to an additional $65 for members and $50 for their spouses and one other dependent (if also covered), every two years.
