The CSA Welfare Fund provides a choice of two dental plans to its members.
Unless otherwise requested, members are enrolled in the SIDS Dental Plan.
- SIDS Dental Program-Allows you to use any dentist. There is a $ 25.00 per person annual deductible. If you use a SIDS participating dentist, the only additional cost is for major work (such as crowns, bridges, and root canals) where there is a $50.00 co-payment. - Healthplex DMO – New York and New Jersey This is a prepaid program, similar to an HMO. You must select a Healthplex primary dentist, and may only use another dentist if referred by your primary dentist. There is no out-of-pocket expense to you with the Healthplex HMO plan
